It's 1830.
Dean has been arrested and charged as mentally ill with 'Aggressive-Omega Syndrome'. Given how out of place anything less then a perfectly submissve omega is one that killed his abusive alpha is almost unheard of.
Almost.
Dean is shipped off to the only omega Prison/Asylum in the world. The island isolated facility, Saint Martha's Home For Mentally Unstable Omegas. Despite the abusive alpha guards and doctors, rough lifestyle, and the occasional genuinely insane omega inmates that are all quite literally driving him crazy Dean has found a home here and cultivated a found family amoung the other omega inmates.
He's even started developing some feelings for his friend and one of the only kind omega nurses Sam Wesson.
He might actually get a chance with him when the omega inmates take over the island and kill every alpha and cruel omega in the place.

Summary
Unable to kill the knights of Hell. (Dean is one of them in this)
Sam binds them to himself. Making it to where they must be within a certain distance of him at all times. They can do him no physical harm and seeing as the spell is tide to his life if he dies of anything other then old age they will never be released from the home he binded them in.
A great incentive for the demons to keep the crippled bastard alive.
While one might think this would be a bitter story about demonic enemies making life Hell for Sam each other and all between you'd be surprise. Everyone adapts eventually, and everyone moves on after a while waiting for the next big event in their lives.
Now you just have a semi- retired mechanic hunter living with 5 weirdly adjusted demons. Splitting chores, watching tv, developing non-murderous hobbies, ordering take out, and making bi-weekly trips together to get groceries.
All the while forgetting they were even waiting for Sam to drop dead so they could leave and get on with theirs live and Dean and Sam even taking a deeper interest in one another then anyone would expect.
It was all so... domestic.
Dean and Sam are not brothers in this.

Aziraphale and Crowley are contemplating moving to get some away time from the constant surveillances of their previous offices.
They are considering America as their next stop but Crowley suspects that eventually that won't be enough either so he's decided to look into some alternative options. Now, he's accepted by now that he'll never convince Aziraphale to go off into the cosmos with him where this won't be a problem, the stubborn bastard likes living near humans too much, however dimension travel may be their second best option.
While looking into their options, as there are few dimensions available out there, Crowley accidentally falls through to one God had no intention of any of her creations, good or bad, going to.
Now, he has to wait in this chaotic monster, demon, and angel infested world while he waits for Aziraphale to find him.

Castiel accidentally falls into another universe.
In a world where people are more physically affectionate naturally. Where you greet strangers with hugs when you first meet them rather then handshakes. Where you kiss loved ones when you see them after a while. Even touch starved hunter children like Sam and Dean would grow up in such a world.

Summary
Sam and Dean relationship has always been far too codependent and just tittering on crossing a line. One night they both get drunk and are a little more happy than the average hunter’s resting state. They finally cross that thin-ass line and have sex.
In the morning, Dean just kind of accepts it as the inevitable happening and is okay with the idea of continuing this. Sam, is not as okay with this and demands they go to therapy to finally work on these issues.
