Watson sighed, but evidently refused to give up. "You had your hands around his neck, Sherlock. You were strangling him." He frowned, leaning down. As if by lessening the distance he could read Sherlock better, could peer into his soul and find whatever it was he was looking for. "Have you ever even hurt a person before?"
"Have you?" Sherlock shot back, already knowing.

During a case, Watson forcefully takes a long walk off a short balcony. Winds up with a concussion and a broken arm for his trouble. Ow. He mostly figures that Sherlock will show the minimum level of sympathy, more concerned that his investigative partner's off the table for the next little while. However, when Mariana gets a call from the hospital, she arrives to find a near catatonic Sherlock Holmes in his hospital room.

Then, self-pity; it occurred to him that he might never see the version of Gregory—indeed, the world—he knew again. He wanted to be angry, but he was just tired. “Please don’t say that,” he said.
“Why not? It’s true.”
“I am not my brother,” Mycroft said. “I do not believe in hearing the truth to the exclusion of all else. Or what you believe to be true.”
