It was one of those questions that was only funny due to how true it was - when Stevie was young, there were no doctors in Schitt’s Creek, and the vast majority of health problems were dealt with by ignoring them or going to Twyla’s family for homemade remedies. Hospitals were always a last resort. Stevie tried not to remember the few times she had been sent to the hospital in Elmsdale. Schitt's Creek didn't need a hospital anymore, they had Ted.
